SC15 Survey Questionnaire

Shell Club 15+ (SC15+) values the protection of the information you voluntarily provide as members of SC15+ and the information accumulated during the various club activities. The club applies reasonable security measures to protect your information in storing and processing your information. To provide the desired service to members, SC15+ needs the members' latest information.

There are 2 reasons for this document, namely, 1) to request your latest information based on the list below, and 2) the club is conducting a survey to provide inputs to our Discussion Series topics.

Below is the list of information that you need to provide so that SC15+ can continue extending its programmed services to you. If you choose not to update your information, the club may take measures regarding support the club can extend to you.

By providing the above information and agreeing to the survey, you consent for SC15+ to process and store the information you provided. The club will store personal and sensitive personal information in a secured cloud-based drive where access is restricted according to the role assigned to the Board of Trustees and the Secretariat. Data will be stored for 5 years from the date the member has withdrawn his/her membership to the club or according to the parameters set by the President of SC15+ in the Records Management Policy.

Members of SC15+ are welcome to view, review and amend the information they have agreed to share with the club in accordance with the rules of the National Privacy Commission (NPC).
